Appointment Checklist
Please bring the following items to your appointment:
- All copays and non-covered services are due in full at the time of service.
- Your insurance card(s)
- A valid driver’s license or photo ID
- For minors: a parent or legal guardian must provide a valid photo ID
- A current list of medications
- Any medical records that may be helpful for your visit
- Plan to arrive at least 15 minutes before your scheduled appointment time
Friendly Reminders
- Complete your patient forms ahead of time. Forms can be found under Forms and Notices below.
- Running late? We do offer a 15-minute grace period if you are running late. We will make every effort to accommodate your schedule. Please call us at 480.947.4493 if you expect to arrive late.
- Cancellation Policy: A $30.00 office visit fee may be charged for any same-day cancellation or no-show appointments.
