Insurance Information

We work with most major insurance providers; however, we do not accept every plan they offer. Please confirm with your insurance carrier that we are in-network with your particular plan. If your insurance changes, let us know before your next visit so we can update your records and avoid billing issues.

Out-of-Network & Self-Pay Patients

If you do not have insurance, or if we do not participate with your plan, you are still welcome at our practice.

Referrals & Authorizations

Some insurance plans require referrals from your primary care physician or prior authorizations for certain procedures. It is your responsibility to ensure that these are in place before your appointment. If you have questions about what your plan requires, our staff can help guide you.

Payment Options

For your convenience, we offer multiple ways to pay your bill:

    • Online: Pay securely through our patient portal.
    • By Mail: Send payment via instructions on your statement. Do not send cash.
    • By Phone: Call our billing office during billing department business hours to pay with CareCredit, credit or debit card.

Your Rights and Responsibilities

As a patient, you have certain rights and responsibilities regarding your medical bills:

    • Right to receive a detailed, understandable bill
    • Right to ask questions about charges
    • Responsibility to provide accurate insurance information
